MAPOLY RE-COMMENCE MEDICAL FOR NEWLY ADMITTED STUDENT

Mapoly reopens medical for all newly admitted students and those haven't done theirs before .

You can now come down to school medical health center (RABIAT SOGA)in the school to obtain your medical form then proceed to the attached medical center .

Required documents are ::

1 ACCEPTANCE FEE (printed from the school fresh student portal )

2 Clearance  form

How to arrange your first stage of registration file(clearance)

**CLEARANCE form**

1.Get your clearance form printed  from your portal on the school website .

2.Fill the olevel required space (with a pen )

3 sign and date (where applicable only ..leave the official space blank please)

4. Affix with gum only(red or white) passport background

5.keep it safe

**ACCEPTANCE FEE RECEIPT(PRINTED FROM FRESH STUDENT PORTAL )

1. Affix with gum only(red or white) passport background .

***HOW TO ARRANGE IT FOR SIGNING BY THE SCHOOL OFFICIALS ***

1. Get your clearance form(attached passport )

2. Acceptance fee receipt (attached passport)

3 change of institution receipt (To all admitted full time admitted prospects only)

4. Jamb result original print out (which your passport shows on it)..(applicable to FULL TIME ADMITTED PROSPECTS ONLY )

3. Original olevel result (statement only to 2015 and neco 2013 only will be allowed )

Note:
Arrange it in that order then staple together .

To all FULL TIME admitted prospects to make your clearance snappy ,you might go to your school officer and HOD for signing if the "QUEUE" is  unbearable .

Proceed to your departmental school officer and HOD
Return to the school admission office opposite jibodu hall in the school for submission of clearance dully signed and stamped by school officer and  HOD of your department ,so as to enable your school office portal !!!!!!!!!.

Keep all documents safe !!!!!!!!!

Note ::
All EVENING /PART TIME admitted prospects are to come down to the school CENTER FOR PART TIME AND DIPLOMA STUDIES (Behind jibodu hall)for school officer sign and stamped but you are to sign with the FULL TIME admitted prospects  at same HOD of your department .
